Emergencies happen to everyone.  When illness or injury strikes, strike back with a LifeCare membership.  The LifeCare Membership Plan is a program approved by the Arizona Department of Health Services that Life Line Ambulance offers to provide unlimited emergency and medically necessary* non- emergency ambulance transportation for you and your family** within our service area*** with NO additional out-of-pocket expenses. The cost is only $49.71 per year, per family!

Annual enrollment dates are September 1-30 only.

* Must meet medically necessary guidelines as defined by CMS (Medicare).  Transport must originate from Life Line Ambulance service areas.
**Covers immediate family, under 21-years old, living in member’s residence.
***Service area is defined by the Arizona Department of Health Services.
